GENERAL REGISTRATION At the beginning of each new academic year, all students, will continue to be enrolled unless written communication is sent to BLMA as described in session 9 of this document. A non-refundable registration fee of $150.00 will be charged per student each academic year or $300.00 per family. New students who register after the beginning of the current academic year are also required to pay the non-refundable registration fee each academic year.

MONTHLY LESSONS – Lessons are held according to our published yearly Teaching Calendar. You will have the same lesson appointment time each week. Students do not pay for holiday weeks (see calendar for details) and summer lessons are included for yearly students. Any permanent schedule change requests must be received in our office seven days in advance of your current lesson day (faculty cannot make schedule changes)

YEAR TEACHING CALENDAR: The calendar is available at the school, in each studio, and included in your registration documents. We are closed for all major holiday weeks as listed on this calendar. Students do not pay for holiday weeks, and these weeks are not included in your tuition.

Tuition: Yearly tuition may be paid in full in advance, or for your convenience may be divided into installment payments due by automatic payment (on the first of each month) through May 1. Tuition installment payments will remain the same each month since holiday weeks have been factored out of your total tuition. In the long months, you will get five lessons, in short months, you will get three lessons, and in most months, you will get four lessons.

The tuition is the SAME each month, i.e., we don’t charge you more when you have five lessons, and we don’t charge you less when you have three lessons in a month, tuition payments are not refundable or transferable. Any changes to an account require a 30 days’ notice from the first of the month. If you drop the lessons prior to July, you might have a balance for the number of lessons you have received.

CLASS/LESSON PREPARATION Students should come prepared for their lesson time with their instrument (except pianist), music, notebook, and pencil. Students should address the teachers in a respectful manner and come with a “ready to learn” attitude.

COMMUNICATIONS BLMA provides a web base Customer/Parent Portal that can be accessed from your computer or phone at any time, type in your browser the following: https://xxx.com//ParentPortal/Login You can check your lessons, make payments, send emails communicating missed lessons, and more. To ensure that all families stay informed BLMA will provide information via emails, our website, as well as posting the information at the academy on the calendar and various bulletin boards.

RECITAL It takes dedication, time, practice, and a large amount of effort from all of us, staff, students, and parents to bring the recital to life. To help you understand some of what is involved please read the following.

All students are required to participate in the recitals, upon the recommendations of their instrument instructor.
Students will perform a recital for each class (instrument or vocal) they are enrolled in.

There will be rehearsal fees associated with the performance. The amount will depend upon how many students are participating.

There may be additional rehearsals for students who are participating. These rehearsals help us ensemble all of the pieces of the recital together. These may occur on a weeknight and/or weekend.

BLMA will charge a per/family rehearsal fee, unless they hold academy membership.

STUDENT LESSON ABSENCES: Please inform the office of any missed lessons to qualify for a Make-up. Missed lessons are not refundable. If a student is ill, they should not attend their lesson. Students missing three or more lessons in a row without notice may be removed from the schedule and can choose a new lesson time when they return. Missed lessons, we must be notified 24 hours prior to your lesson time to qualify for Make-up lessons, this communication must be in the form of an email or text (written notification only).

These are the make-up options:
Online zoom lessons can be set up on the same day and time as your lesson
If the student is not available for the zoom online lesson and the lesson was canceled up to 24 hours prior to the lesson time, the instructor will send a pre-recorded lesson that can be accessed from the comfort of your home at any time.

We offer many one-hour group workshops during the year for students who miss a lesson and didn’t qualify for zoom or pre-recorded lessons.

TEACHER ABSENCES: Occasionally, it may be necessary for your teacher to be absent due to illness, performances, etc. In this case, another instructor will substitute. If a sub is not available, you will be contacted to reschedule the lesson.
Make-up for elective Lessons/GROUP (music theory, solfege, keyboard lab, conducting, chamber music, string orchestra) BLMA does not offer make-up for elective lessons if students missed a class. 11. LATE ARRIVAL: Please make every effort to arrive at your scheduled lesson on time or call if you see that you will not be on time. If you arrive late, your instructor will do their best to work within the time that is left. However, please understand that due to scheduling constraints, we will not extend the lesson beyond our original schedule time. No make-up lessons will be given for late arrival. Teachers are not required to stay on campus past half of a missed lesson time.

12. STUDENT ILLNESS: The school follows the same policy as the regular schools regarding illness. If a student has not attended school for the day due to illness, they will not be able to attend their lesson at BLMA as well. Please keep your child at home if they have symptoms of illness such as fever, rash, runny nose, sneezing, vomiting, diarrhea, body aches, etc. To return to lessons it is asked that the student be free of symptoms for 24 hours. Germs are passed very easily, and we want to keep our students and teachers well! Your help with this is appreciated!

13. DISCONTINUING LESSONS: Most students are with us for years. If you cannot continue lessons, then we will certainly be “Sad-to-see-you-go”. There is no long-term contract. To withdraw and suspend automatic tuition charges, for one full month (30 days) prior notification from the first of the month is required, even if the first of the month is a school holiday. The ” Sad-to-see-you-go ” form is available in our office, withdrawing by phone is not acceptable. Tuition is not refundable. If a one-full-month notice is not provided, the fees and outstanding tuition (including tuition fees for the entire month) are still due and payable and will charge upon withdrawal notification. The last day to withdraw is May 1st.

14. ETHICAL PRACTICES: Please respect the school’s investment in the training and goodwill of our faculty. In return for Vivace providing instruction to students under the terms and conditions set forth herein, students agree not to, directly or indirectly, hire or retain the services of the school’s teachers for instruction in a student’s home or outside BLMA during the period of instruction and for 12 months thereafter. Students who do not honor this provision agree to pay a placement fee to BLMA in an amount equal to the average weekly fee received or to be received by BLMA from the student for instruction multiplied by 44, BLMA and the student agree on the amount of the placement fee shall be calculated on a per-student basis and represents the total damage per student recoverable by BLMA if there is a breach.

15. PAYMENT & BILLING All payments are processed by pre-authorized credit card Visa, MasterCard, American Express, Discover, or Debit Card on the 1st (first) day of each month. Tuition that is not paid on time for any reason will incur a late fee service charge of $25. Tuition is never prorated for missed lessons. Tuition does not include any retail teaching materials such as books or instruments.

I have received and read BLMA’s Handbook.

I have had a reasonable opportunity to ask questions about it and understand its contents.

I am responsible to know the contents of this Handbook, including BLMA’s rules and regulations.

I understand that this Handbook has been prepared to give me information regarding BLMA and its general policies and expectations.

The policies in this Handbook are merely guidelines that may be changed, interpreted, or discontinued at any time with or without notice.

(I/We) do hereby authorize the BLMA, hereinafter name the Company, to initiate recurring (debit or credit) entries to (my/our) (Credit Card Account) as indicated and named below as the depository financial institution, hereafter named Financial Institution. (I/We) acknowledge that the origination of ACH transactions to (my/our) account must comply with the provisions of the U.S. law. Furthermore, if any such debit(s) should return NSF, (I/we) authorize the Company to collect such debit(s) by electronic debit and subsequently collect a returned debit NSF fee of $25.00 per item by electronic debit from my account identified below, and authorize all of the above as evidenced by my signature below. I understand that if I wish do discontinue payments being charged, I must send BLMA, BL MUSIC ACADEMY, INC. a signed note stating that I wish to discontinue this plan or change payments amounts. If I do neither and just stop coming to classes, my account will be continued to be charged until I communicate in written. I understand that if my account status changes, I should notify BLMA, BL MUSIC ACADEMY, INC. before the 15th of your last month. If I neglect to update my account, I understand that I am liable for any fees incurred, including a $25 late fee after the 5th day of the month. By signing below I agree to all stated on this form.

This Release is executed this date______________ , by______________________________ (“Releasor”). In consideration of being permitted to participate in programs/activities conducted by BLMA, BL MUSIC ACADEMY, INC., Releasor, for himself/herself and his/her personal representatives, heirs and next of kin, releases, waives, discharges and covenants not to sue BLMA, BL MUSIC ACADEMY, INC., its officers, directors, board members, supervisors, agents, servants or employees (collectively “Releasee”) from any and all liability to the Releasor, his/her personal representative, assigns, heirs and next of kin for loss or damage, and any claim or damage therefor, on account of injury to the person or property resulting in death of the Releasor, whether caused by the negligence of Releasee or otherwise while the Releasor is involved in programs, activities or any other purpose at BLMA, BL MUSIC ACADEMY, INC. Releasor agrees to indemnify Realeasee from any loss, liability, damage or cost Releasee may incur due to the presence of Releasor in or on BLMA, BL MUSIC ACADEMY, INC., , Hialeah, Florida, 33012 (the “Premises”), whether caused by the negligence of the Releasee or otherwise. Releasor assumes full responsibility for and risk of bodily injury, death or property damage due to negligence of Releasee or otherwise while in or on Premises and/or while competing, officiating in, working or for any purpose participating in programs and activities for BLMA, BL MUSIC ACADEMY, INC. Releasor agrees that this release, waiver and indemnity agreement is intended to be as broad and inclusive as permitted by the laws of the State of Florida and that if any portion of the agreement is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. Releasor, being of lawful age, in consideration of being permitted to participate in BLMA, BL MUSIC ACADEMY, INC.’s programs and activities, does for himself/herself, his/her heirs, executors, administrators and assigns, now releases and forever discharges Releasee, their heirs, administrators and executors of and from any and every claim, demand, action or right of action, of whatsoever kind or nature, either in law or equity arising from or by reason of any bodily injury or personal injuries known or unknown, death and/or property damage resulting or to result from any accident which may occur as a result of participation in BLMA, BL MUSIC ACADEMY, INC.’s programs and activities or any activities in connection with BLMA, BL MUSIC ACADEMY, INC., whether by negligence or not. Releasor further releases Releasee from any claim whatsoever on account of first aid, medical treatment or service rendered him/her during participation in BLMA, BL MUSIC ACADEMY, INC.’s programs and activities. I understand that this Release applies to all future visits to the Premises in addition to all future BLMA, BL MUSIC ACADEMY, INC. programs and activities. I agree to arbitrate all matters that arise out of this Release.

 

I, _____________________________, certify that I am the parent or legal guardian of ________________________ (“Minor”), who is a minor. In consideration for my child being able to participate in the programs and activities offered at BLMA, BL MUSIC ACADEMY, INC., I agree on my own behalf and on behalf of Minor to release, waive, discharge and covenant not to sue BLMA, BL MUSIC ACADEMY, INC. along with its officers, directors, board members, supervisors, agents, servants or employees (collectively referred to here as “Releasee”) from any and all liabilities, claims, demands or causes of action that may arise from or be related to any loss, damage or injury, including death that may be sustained by Minor or Minor’s personal property while Minor is participating in BLMA, BL MUSIC ACADEMY, INC.’s programs or activities or while Minor is on its premises located at _______________________________(the “Premises”). I further release, on my own behalf and on behalf of Minor, BLMA, BL MUSIC ACADEMY, INC. from claims for negligence. To the best of my knowledge, Minor is in good physical condition, and I have no knowledge of any physical condition, injury or illness whatsoever which would place Minor at risk to participate in the BLMA, BL MUSIC ACADEMY, INC.’s programs and activities. I am fully aware of the risks connected with participating in the programs and activities at BLMA, BL MUSIC ACADEMY, INC.. I voluntarily assume full responsibility for any risk of loss, property damage or personal injury, including death, that Minor may sustain as a result of participating in BLMA, BL MUSIC ACADEMY, INC.’s programs and activities, however caused. I further agree to indemnify and hold harmless Releasee from any loss, liability, damage or cost, including court costs and attorneys’ fees, that may accrue related to Minor’s participation in BLMA, BL MUSIC ACADEMY, INC.’s programs and activities, however caused. While Minor is attending BLMA, BL MUSIC ACADEMY, INC., I give permission for the staff of BLMA, BL MUSIC ACADEMY, INC. to administer appropriate medical attention to Minor in the event of any accident, illness or injury. I will be responsible for any and all costs of medical care and treatment that may be provided, except for care and treatment covered by my insurance. This instrument shall be binding upon the members of my family, my spouse and my heirs, assigns and personal representatives. This Release and disputes arising thereunder shall be governed by the law of the State of Florida. I understand that this waiver and consent form applies to all future visits to the Premises in addition to all future BLMA, BL MUSIC ACADEMY, INC. programs and activities. I agree, on my own behalf and on Minor’s behalf, to arbitrate all matters that arise out of this Release. This Release contains the entire agreement between the parties to this agreement, and the terms of this Release are contractual and not a mere recital. I certify that I have read and fully understand the above Release. I certify that I am signing this Release freely and voluntarily and that I understand that by signing this

Release executed by________________________________ (“Releasor”) in favor of BLMA, BL MUSIC ACADEMY, INC. (“Releasee”). In consideration for participation in BLMA, BL MUSIC ACADEMY, INC.’s programs and activities, Releasor consents to being the subject of the photographs of Releasee, together with any subject matter owned by Releasor, and authorizes Releasee to cause the same to be exhibited, with or without advertising sponsorship, as still photographs, transparencies, motion pictures, television, video or other similar media. Releasor releases Releasee, and any associates, as well as any and all officers, directors, board members, assigns, agents, servants and employees from any and all claims for damages for libel, slander, defamation, invasion of privacy or any other claim based on use of the above-described materials.

SALE TERMS FOR MUSICAL INSTRUMENTS AND ACCESSORIES

All orders placed on blmusicacademy.com are pre-paid via Credit Card. All orders placed with BL MUSIC ACADEMY INC. are pre-paid via Credit Card and paypal account.

All cancellations are subjected to BL MUSIC ACADEMY INC.. approval. Cancellations will not be accepted for work in progress, re-label production, or private label orders.

Although it is uncommon for BL MUSIC ACADEMY INC. to cancel an order, we reserve the right to deny any order from a customer where the following may occur:

 

Billing and Shipping Information:

Orders cannot be processed if an information error is found. These include the following:

  • Fraudulent information
  • Incorrect shipping address
  • Incorrect billing information and address
  • Incorrect credit/debit card information, including number, expiration date, or security code

 

Shipping charges for orders placed at blmusicacademy.com are calculated through USPS directly. BL MUSIC ACADEMY INC. does not calculate these shipping rates.

Delay of Shipment/Back Order:

If a product is on back order and has not yet been received in our warehouse, your order may be delayed. Once the warehouse receives the product, your order will be shipped immediately.
